Build a Complete Medicine and Supplement List for Your Pain Visit

A medication list is most useful when it shows what you actually take—not only what appears in an old chart. Before a pain-management visit, gather prescriptions, over-the-counter products, vitamins, herbal products, creams, patches, and anything used only as needed.

Write down the essentials

For each item, include the name, strength, amount, time of day, and why you take it if you know. Mark anything you recently started, stopped, or changed. Do not guess: check the label or bring the original container when practical.

Include reactions and questions

If a product caused a problem, describe what happened and when. “It made me dizzy for two hours” is more useful than labeling something an allergy without details. Add any question about timing, refills, interactions, or confusing directions.

Keep every clinician informed
