
If you have had imaging, injections, procedures, or prior pain-management care elsewhere, organizing a few key records before your visit can make the appointment more productive.
Start with the most recent information
Bring recent imaging reports, a current medication list, and the latest pain-management documentation you can obtain. Older records may still matter, but the most recent information is usually the easiest place to start.
Write down what is missing
If you know a report or note exists but do not have it yet, list where it was completed and the approximate date. That can make follow-up easier after the visit.
Keep the packet simple
You do not need to arrive with every medical record you have ever had. Focus on documents related to the problem being evaluated and any recent changes in care.
